Menstrual pain, which is said to be experienced by nearly 50% of women of childbearing age, is divided into primary menstrual pain that occurs without any underlying disease and secondary menstrual pain that occurs in the uterus. It doesn’t just stop at abdominal pain; it also causes pain in the pelvis, lower back, and tailbone, and can also be accompanied by symptoms such as indigestion, constipation, diarrhea, and edema. If your pain does not improve even after taking painkillers, endometriosis may be the cause.
■ What is endometriosis?
Endometriosis is a disease in which the endometrium tissue, which is supposed to cover the inside of the uterus, is located in areas other than the uterine cavity. It occurs in approximately 10-15% of women of childbearing age. It can occur in any woman of childbearing age, from menarche to menopause, and is a common cause of infertility because it reduces ovarian function.
■ Causes of endometriosis
The exact cause of endometriosis is not known, but reflux of menstrual blood is considered a likely cause. Menstrual blood drains through the vagina, but some flows back through the fallopian tubes and enters the abdominal cavity. This can happen to almost all women. If the uterus is healthy, it is removed by the immune system, but in some women, it is not removed normally and grows and causes endometriosis. A family history of endometriosis, an imbalance in follicular hormones among female hormones, a short menstrual cycle of 27 days or less, a long menstrual period of more than 7 days, or heavy menstrual flow may affect the development of endometriosis.
■ Endometriosis symptoms
The most common symptoms of endometriosis include severe menstrual pain, premenstrual syndrome, chronic pelvic pain, and pain during sexual intercourse. There are many cases where women go to the hospital because they are having trouble getting pregnant and are diagnosed with endometriosis. Irregular bleeding and pain during bowel movements may also occur. If you experience the following symptoms, you must visit an OB/GYN and get tested for endometriosis.
ㅁ Pain in the lower abdomen or back extends to the legs
ㅁ It feels like the bottom is falling out.
ㅁ It hurts like being stabbed in the pelvis with a needle.
ㅁ It is difficult and painful to have a bowel movement during menstruation.
ㅁ The pain in the lower abdomen is particularly severe, and regular painkillers do not work.
If menstrual pain begins 36 to 48 hours before menstruation and continues for several days after menstruation ends, if menstrual pain does not occur for several years after the first menstruation but suddenly occurs after the age of 20, or if menstrual pain lasts for more than 5 to 6 days every menstruation Inspection is also required.
■ Endometriosis treatment
Although endometriosis does not lead to cancer, it causes severe pelvic pain, and as the disease progresses, it adheres to the uterus, ovaries, or intestines, destroying the normal organ structure and causing infertility. Endometriosis has a variety of symptoms, ranging from asymptomatic to severe pain, depending on the location and progress of the lesion. Therefore, even if a blood test or ultrasound test is performed, an accurate diagnosis may be difficult. If endometriosis is suspected, visual observation through laparoscopy is the most reliable diagnostic method. Since recurrence is frequent, endometriosis is removed through precise surgery followed by drug therapy.
Endometriosis is a disease closely related to menstruation. You need to pay close attention to your menstrual cycle, amount, and menstrual pain. About one-third of patients do not have any special symptoms, and endometriosis can occur even if menstruation is regular. Therefore, if you are a woman of childbearing age, please get a regular check-up at an obstetrics and gynecology department once a year.
