Insurance claim, personal information processing consent form
Hospital documents, accident confirmation documents
Copy of bankbook in the name of the beneficiary

How to cancel AIA Life Insurance and required documents
Termination of normal contract
Mail application: Paper payment claim, contractor’s seal, seal certificate, bankbook copy, customer transaction confirmation
In-person application: Contractor’s ID card, bankbook, and additional documents required for agents
Online counter: Use the menu [MY AIA – My Insurance Information – Check Contract Details]
Termination of effective contract
